Job Title: Clinical Pharmacologist

Location: Unit

Job Summary: Responsible for investigating and recommending the appropriate usage of drugs within the Hospital; teach rational drug use at all levels.

Business/ Finance

  • Monthly report collation of various medication errors, adverse drug reactions, prescription audits and crash cart audits.
  • Responsible for preparing quality indicators.

People

  • Educating all medical and paramedical staff about medicines.
  • Attending the drug therapeutic meeting on monthly basis.
  • Guide the team in auditing all the indents to anticipate, prevent, and investigate medication errors of all types.
  • Coordinate with doctors and suggest dose adjustments in cases of abnormal pharmacokinetics and pharmacodynamics of drugs
  • Coordinate with doctors and nurses and answer queries related to proper use, storage, stability and over dose of drugs
  • Conduct periodical training to ward/ICU pharmacist and nurses (if required) to upgrade their knowledge base

Customer

  • Coordinate with pharmacy purchase and consultants on purchase of medicines.
  • Addressing the patient queries in case of allergy & drug reactions.
  • Coordinate with companies on recall of medicines.

Operations

  • Taking rounds in all inpatient units to monitor drug administration, prescription and drug interactions.
  • Taking various internal audits to ensure quality of patient care with respect to medication usage
  • Monitor medication errors (requisition, dispensing, administration, prescription and documentation).
  • Evaluating root cause analysis with corrective and preventive actions of medication errors.
  • Audit the prescription from patients case file to check if the medicines of individual patients are appropriate, safe and effective and give recommendation to doctors on how to resolve or minimize any drug related problems.
  • Monitoring storage of medications across the hospital like high alert medications, concentrated electrolytes, cold chain medications etc.
  • Answer queries from medical, nursing and pharmacy staff about medication management.
  • Collaborate with medical team to prepare/revise medication treatment protocols and guidelines as and when needed.
