JOB SUMMARY

The RN is responsible for maintaining optimum standards of nursing care and practice through pre procedure, intra procedure and post procedure assessment, planning, implementation, and evaluation of patient care.

NURSING PROCESS

  • Provide direct clinical services for patients, according to established protocols and utilizes evidence-based clinical practice standards under the supervision of the unit head and departmental head.
  • Receives the patient along with the medical records and checks whether all the pre procedure instructions are carried out.
  • Explain the necessity, preparation, nature and anticipated effects of procedure(s) to patients and patient's family members.
  • Conducts an individualized patient assessment / screening, prioritizing the data collection based on the patients immediate condition or needs
  • Documents patient assessment findings
  • Reduces fear and anxiety of the patient by communicating in a gentle manner.
  • Prepares and maintains Cath lab for all procedures.
  • Establishes and utilizes a checklist to monitor the equipment for conformity to all standards for ensuring safety for the patient and all personnel.
  • Assembles the type and quantity of material and special equipment needed for daily procedures and possible emergencies.
  • Circulates and scrubs for the all the Cath lab related procedures.
  • Demonstrates skill in identifying instruments as per the procedure, checks cleanliness and proper functioning of instruments, packs and sends for autoclaving / gas sterilization.
  • Demonstrates skill in assisting the doctor for the procedure.
  • Ensures the patient is cleaned after applying the dressing and provides a comfortable position.
  • Is knowledgeable and competent to watch for hematoma and pedal pulse.
  • Documents vital signs and nursing intervention during the procedure and after the completion of the procedure.
  • Ensures the patient is shifted safely to the recovery room
  • Demonstrates ability to identify arrhythmias and cardiac arrest
  • Takes specimens and blood samples for testing.
  • Informs the laboratory reports to the physician immediately
  • Initiates cardiopulmonary resuscitation or other emergency response activities and codes in case of a medical emergency
  • Evaluates and carries out physician's orders ensuring that they are appropriate such as not causing allergic reactions and given in proper dosages.
  • Coordinates with the ward secretary regarding admission, discharge and transfer of patients; and billing related issues.
  • Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient and family’s diversity, preserving their autonomy, dignity and rights, and is culturally competent.
  • Reports patient’s condition to appropriate personnel during each shift.

QUALITY CONSCIOUS

  • Updates and keeps abreast of the HAI statistics and control measures and practices them in the working environment.
  • Demonstrates skill in preventing medication administration errors, hematoma and patient falls
  • Safely uses the sharp items and disposes the sharps properly including all the other biomedical wastes
  • Works as a team to enhance patient satisfaction and physician satisfaction.
  • Ensures cost effective usage of organization’s resources in the delivery of patients’ care.

MEDICATION MANAGEMENT

  • Is knowledgeable about the medication process and ADR
  • Accurately transcribes medication and treatment orders for the indenting process.
  • Demonstrates skill in handling cardiac drugs and emergency drugs
  • Accurately and safely administers prescribed medications and treatments using correct methods of administration and adheres to policy (7 R’s of medication, 2 identifiers, allergic history, double check for applicable medicines), admixtures, outside medication, self-medication policy, Verbal order policy etc.)
  • Accurately maintains medications administration record.
  • Demonstrates safe storage and control of medication
  • Immediately reports any medication error, ADRs and accurately completes incidents report or ADR forms.
  • Evaluates and carries out physician's orders ensuring that they are appropriate such as not causing allergic reactions and given in proper dosages.

INFECTION CONTROL

  • Is knowledgeable on 5 moments of hand hygiene
  • Performs hand hygiene using 6 steps
  • Segregates waste as per the protocol
  • Notifies the infection control department about the notifiable diseases
  • Follows strict infection control protocols and takes responsibility in ensuring quality control related to quality monitors.
  • Ensures appropriate cleaning and sterilization of the Lab after infected cases

PROFESSIONAL GROWTH AND DEVELOPMENT

  • Utilizes effective communication skills in reporting pertinent information on time.
  • Adheres to the hospital policies.
  • Awareness of the Hospital and Departmental vision, mission, objectives and its implementation.
  • Participates in the hospital quality improvement program.
  • Maintains confidentiality of patient information.
  • Adheres to the safety protocols of the hospital and ensures safety of the patient.
  • Works in accordance with the NABH / JCIA standards.
  • Ensures appropriate cleaning and sterilization of the Cath lab after infected cases.
  • Follows strict infection control protocols and takes responsibility in ensuring quality control related to quality monitors.
  • Identifies educational needs of the family and participates in teaching opportunities. Provides individualized patient/family education customized to the patient and his/her family needs.
  • Performs and participates in quality/performance improvement activities and clinical research.
  • Demonstrates responsibility for professional practice through active participation in continuing education. Enhances the body of knowledge of nursing or area of specialization through exchange of ideas and knowledge in CNE sessions and other training sessions
  • Possess BLS, ACLS certification
  • Demonstrates competency in NABH / JCIA Mandates, Fire Safety, Infection Control, Radiation Safety, HAZMAT safety and Proper use of biomedical equipment

  • Is knowledgeable of patient rights and responsibilities and treats all patients with dignity and respect in a consistent and caring manner.
  • Understands the need for and maintains appropriate confidentiality at all times when interacting with patients, family, visitors and all other contacts
  • Exhibits excellent customer relations skills as evidenced by supportive and constructive communication with all contacts including co-workers, patient, visitors, and families.
